I hereby grant HF Shooting, LLC permission to the rights of my image, likeness, and sound of my voice as recorded on audio or videotape captured on my registered class date without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising from or related to the use of my image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.
Photographic, audio, or video recordings may be used for the following purposes:
• Conference presentations
• Educational presentations or courses
• Informational presentations
• Online educational courses
• Educational videos
• Advertising media
By signing this release, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in a public setting. I will be consulted about the use of the photographs or video recordings for any purpose other than those listed above. There is no time limit on the validity of this release, nor is there any geographic limitation on where these materials may be distributed.
This release applies to photographic, audio, or video recordings collected as part of the sessions listed on this document only. By signing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for:

I understand that it is my responsibility to remove myself from photographs of others where I am incidentally present in the background. I will take the initiative to be mindful of the situations in which the media teams or other representatives of the facility or training team may be taking video or photographs.

Liability & Indemnification
WHEREAS, in return for instruction in firearms, use of premises, and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the Undersigned agrees to the following: The Undersigned agrees to indemnify, hold harmless and defend HF Shooting, LLC and any of its employees, directors, officers, or agents (hereinafter referred to as HF Shooting, LLC), from any and all fault, liabilities, costs, expenses, claims, demands, or lawsuits arising out of, related to, or connected with: the discharge of firearms; the course of instruction; the Undersigned’s participation in the course of instruction; the range, buildings, land, and premises used for the course of instruction (hereinafter the “Premises”); the Undersigned’s presence on or use of said Premises; and any and all acts or omissions of the Undersigned.
And should any such claim, demand, or lawsuit arise or be asserted in any way whatsoever related thereto, whether arising under the laws of the United States or of any State, or under any theory of law or equity, the Undersigned will indemnify, hold harmless, and defend HF Shooting, LLC from any and all costs, expenses, or liability including, but not limited to, the cost of any settlement or judgment made or rendered against HF Shooting, LLC, whether individually, jointly, or in solido with the Undersigned, together with all costs of court and other costs or expenses incurred in connection with any such claim, demand, or lawsuit, including attorney’s fees.
The Undersigned furthermore waives for himself/herself and his/her executors, administrators, assignees, or heirs, any and all rights and claims for damages, losses, demands, and any other actions whatsoever, which he/she may have or which may arise against HF Shooting, LLC, (including, but not limited to any and all injuries, damages, or illnesses suffered by the Undersigned or the Undersigned’s property), which may, in any way whatsoever, arise out of, be related to, or be connected with: the course of instruction; the Premises, including any latent defect in the Premises; the Undersigned’s presence on or use of said Premises; the Undersigned’s property (whether or not entrusted to HF Shooting, LLC); and the discharge of firearms.
HF Shooting, LLC shall not be liable for, and the Undersigned, on behalf of himself/herself and his/her executors, administrators, assignees, or heirs, hereby expressly releases HF Shooting, LLC from any such claims. The Undersigned hereby expressly assumes the risk of entering the Premises and of taking part in activities on the Premises, which include, but are not limited to, instruction in the use of firearms, the discharge of firearms, and the firing of live ammunition.
Furthermore, the Undersigned hereby acknowledges and agrees that he/she has read, understands, and will at all times abide by all HF Shooting, LLC range rules and procedures. This instrument binds the Undersigned and his/her executors, administrators, assignees, or heirs.
I understand that firearms training is potentially dangerous and I release, waive, and indemnify HF Shooting, LLC, including its employees and owners, from any accidents, injuries, damages, or death during the training classes.
ITAR & U.S.C. 922 AGREEMENT
An Amendment to the Gun Control Act of 1968 (18 U.S.C. 922) makes it a felony for anyone who has been convicted of a misdemeanor crime of domestic violence to ship, transport, possess, or receive firearms or ammunition. It is also a felony for any person to sell or otherwise dispose of a firearm to any person so convicted. A person shall not be considered as having committed a “crime of domestic violence” for purposes of the firearm restriction of the Gun Control Act unless all of the following elements are present:
(1) The person was convicted of a crime;
(2) The offense has as its factual basis the use or attempted use of physical force, or threatened use of a deadly weapon;
(3) The convicted offender was at the time of the offense:
a. A current or former spouse, parent, or guardian of the victim,
b. A person with whom the victim shared a child in common,
c. A person who was cohabitating with or has cohabitated with the victim as a spouse, parent, or guardian, or
d. A person who was similarly situated to a spouse, parent, or guardian of the victim;
(4) The convicted offender was represented by counsel or knowingly and intelligently waived the right to counsel;
(5) If entitled to have the case tried by jury, the case was actually tried by jury, or the person knowingly and intelligently waived the right to have the case tried by jury;
(6) The conviction has not been expunged or set aside, and the convicted offender has not been pardoned for the offense or had civil rights restored, or the pardon, expungement, or restoration of civil rights provides that the person may not ship, transport, possess, or receive firearms.
Following U.S. Code 922, the above-described factors of disqualification for firearms ownership, possession, or training do not apply to me.
It is in the interest of HF Shooting, LLC to ensure that all participants in training comply with Federal Law. To ensure compliance, the following acknowledgment must be completed and signed by all individuals participating in HF Shooting, LLC events involving the use or possession of firearms or ammunition.
